If valid blood test is not submitted the athlete will not be allowed to compete per Louisiana Athletic Commission. Athletes under 18 no blood test is required.(Required)

I give permission to the Texas Muaythai and Kickboxing Association to publicly post my name, my registration form information, any photos or likeness supplied by me, captured by an event photographer, videographer, attendee and/or taken at a TBA-SA sanctioned event. ALL of the listed may be used, with my permission, on their website, on social media pages, in documentaries, and/or on marketing materials and in advertisements.(Required)
